CFYS Emergency DSS Box Pickup Request
This form is only for neighbors experiencing a food emergency, diaper emergency or formula emergency in between our food pantry and diaper hub distributions. Neighbors are allowed to pick up an DSS emergency food box, diapers and/or formula once per month.
What's in the DSS Emergency Box? Boxes will be distributed based on your household size.
1 box of Cereal, 1pack of powdered nonfat milk, 1 can of mixed fruit, 1 can of tuna, 1 pack of spaghetti noodles, 1 can of spaghetti sauce, 2 cans of vegetables.
